Getting started with the Prana Group is easy, and this section can help! 

The first step is to call the office at 613.230.0909 to book an appointment, and then you can get a head start by completing the initial paperwork in advance of your visit.

Your health and well-being are influenced by many factors, so in order to get the most complete picture of your current state of health, we first need some information about your health history.

Filling out these forms with as much detail as you can, as well as answering any questions the doctor may ask you during your initial visit will help us draw accurate conclusions about your health challenges, and the most effective way to help you achieve your health goals. 

Your responses will also help us identify the underlying causes of your health issues and formulate a treatment plan customized to meet your individual needs.

Before your first visit, please download the ‘New Patient Intake Form’ so you can print it off, fill it out and bring it to your appointment. 

If you are coming to Prana for pregnancy or post -partum treatment, please fill out that form as well.  If you are bringing your child to Prana for the first time, please fill out the Pediatric Intake Form.
